Rates
$60.00 per 50-minute session for Individual Sessions - Private Pay; $70.00 per 50-Minute session for Individual Sessions if Court Ordered [Includes report for court}; Family Therapy - 90.00 per 50-minute session [$100.00 if court ordered which includes report for court]. Sliding scale for low-income clients. Accepting Out of Network Clients & CPS Clients. If you are pending going to court a fee of 100.00 per hour will be required in advance and you will need to sign the agency policy prior to beginning therapy for me to work with you and preparing you to go to court. Insurance companies do not pay for me to go to court so this will allow me to close my office and spend the day with you in court.
Fees for Relaxation lessons are $10.00 for individual and $5.00 for groups, Workshops for Relaxation is $25.00 and includes all materials needed.
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Reduced Fee
Special Military Rates. All Military personnel, to include Active Duty, Retired, Reserve or any form of Veteran honorably served or can explain Other than honorable service the fee will be based on Rank and Grade or ability to pay if unemployed.
Payment
Cash, money orders and all major credit cards accepted for payment. Traditional Medicaid, Superior Health Care, Blue Cross Blue Shield, United Health Care, CPS and Tricare are accepted.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session. If you schedule and do not show up for three consecutive appointments I will terminate working with you.
